Luxury Veil Concierge LLC is committed to protecting the privacy of health information shared with us in the course of providing care. This notice describes how health information about you may be used and disclosed, and how you can access that information.

Unlike the general contact form on this website, which does not request medical details, protected health information (such as your procedure, recovery needs, and relevant medical history) is collected directly and securely during your private consultation and throughout the course of your care.

• For payment and standard healthcare operations related to your care
• As required by law or in response to a valid legal request

We do not use or disclose your health information for marketing purposes without your written authorization.

We are required by law to maintain the privacy of your health information, provide you with this notice of our legal duties and privacy practices, and abide by the terms of the notice currently in effect.

If you believe your privacy rights have been violated, you may file a complaint with us at the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized for filing a complaint.

We reserve the right to change this notice and to make the revised notice effective for health information we already have as well as information we receive in the future.
